What do I Need to do Now?

If, after having read all the information and discussed it with your family, you have decided to bequeath your tissue to Parkinson’s disease research and none of your family have raised objections, then you and your next-of-kin will need to complete and return the three forms enclosed in the information pack. See below for information on how to request a pack.

We will keep your original registration forms and send you photocopies for your records. We will also send you your Tissue Bank Donor Card. Always carry your card so that in an emergency staff at a hospital will know immediately of your bequest to the Tissue Bank.

The Forms

If you require further information, or wish to register as a donor, please fill out a request for information form. This can be filled in online and submitted electronically. Alternatively, the request form is available for download in a variety of formats. (See below.)

The PDF version is better for printing out and filling in by hand, but requires you to have Adobe Acrobat Reader installed on your machine. (This software can be downloaded free of charge from the Adobe website.) The MS Word, RTF and text versions can either be filled in electronically, or printed off and completed by hand. The completed form can be returned to the Tissue Bank by e-mail, by fax, or by post.
